Mollom
I personally use mollom.
It doesn't filter everything but it helps. I then have to check the queue and publish the real comments.

we already use mollom for months now..
it is really helping, but we still have issues with somethings like user registration and so..

The main point is that we have human spammers, all automated tools such as captchas won't stop them..
IMHO we need new policies, like making the new user unable to post links in his first 3 posts, or denying him to put links in his own profile in the first day or registration, ... etc.

Put links comments in moderation.
Yes, every spam needs to have at least one link that is leading to some spammer site. I am presuming that the human spammer always put at least one link unlike the others *normal humans* that wants from time to time to write a comment including links or something.
This module will check newly added comments. If a link has been found in the comment, the module will put that comment in moderation and send a notification to site admin.
I also recommend mollom.
I like this module
its idea is simple and should be efficient, anyway we can't do it now as it's a drupal6 module, and instead of backporting it, we are planning to upgrade to drupal6 soon isA.
but this only works for comments, may be we need something more than just comments.. sometimes they do new posts, and the main hit we have now is spammer users registered in our site and putting their spam links in the user profile..

I don't like that idea
I don't like my comments to be delayed to be moderated, this will stop the flaw of discussions (if there's a flaw)
